NDIS Support Coordination for Psychosocial Disability — Gold Coast

Psychosocial disability is one of the most misunderstood areas of the NDIS. ConnectCare provides grounded, practical support coordination for participants with mental health conditions across the Gold Coast.

What Is Psychosocial Disability in the NDIS Context?

A psychosocial disability arises from a mental health condition — such as schizophrenia, bipolar disorder, severe depression, PTSD, or anxiety disorders — when the condition significantly and persistently impacts a person's ability to participate in daily life and the community.

The NDIS funds psychosocial disability under the same framework as other disability types. However, psychosocial plans often have more moving parts — multiple providers, complex service agreements, and a need to coordinate with mental health treating teams — making support coordination especially valuable.

Challenges Participants Face

  • Difficulty engaging with services during low periods or episodes
  • Multiple providers (psychiatrist, psychologist, case manager, NDIS) operating in silos
  • Plans that don't reflect real-world needs or daily fluctuations
  • Stigma when navigating mainstream services or support worker relationships
  • Frequent plan reviews or changes due to evolving mental health status
  • Lack of local Gold Coast providers who genuinely understand psychosocial disability

How Coordination Helps with Mental Health-Related Plans

A support coordinator who understands psychosocial disability acts as a stabilising presence — maintaining consistent contact with the participant, liaising with their treating team (psychiatrist, GP, psychologist), and making sure providers are communicating and working toward shared goals.

ConnectCare helps participants during plan reviews to articulate how their mental health affects their functional capacity — which is often the missing piece in underfunded psychosocial plans. We also connect participants with community participation supports that build social connection, which is a strong evidence-based protective factor for mental health.
