NDIS Support Coordination for Acquired Brain Injury (ABI) — Gold Coast

ABI creates some of the most complex support needs in the NDIS. ConnectCare coordinates across medical teams, families, and multiple providers to ensure participants with acquired brain injury receive the integrated support they need on the Gold Coast.

ABI-Specific NDIS Navigation

Acquired brain injury (ABI) includes traumatic brain injury (TBI), stroke, hypoxic brain injury, encephalitis, and other conditions that affect the brain after birth. The functional impacts are wide-ranging — affecting memory, communication, behaviour, physical mobility, fatigue levels, and emotional regulation.

ABI participants often require the highest level of support coordination because their needs are complex, evolving, and span multiple care domains simultaneously. An NDIS plan that doesn't capture the true complexity of an ABI will be chronically underfunded. ConnectCare knows how to build and document a plan that reflects genuine need.

Complex Support Needs

ABI support commonly spans personal care, behaviour support, cognitive rehabilitation, community participation, communication assistance, and assistive technology. ConnectCare coordinates across all of these areas:

  • Personal care workers experienced with ABI-specific physical and behavioural needs
  • Behaviour support practitioner referrals and coordination
  • Communication device and assistive technology navigation
  • Community participation supports that account for fatigue and cognitive load
  • Coordinating occupational therapy, speech pathology, physiotherapy, and neuropsychology
  • Transport and mobility support coordination

Working with Families and Medical Teams

ABI almost always involves family members or carers who are deeply invested in the participant's recovery and ongoing support. ConnectCare includes families in the coordination process — keeping them informed, inviting them to planning conversations, and supporting them to understand what the NDIS can and cannot fund.

We also liaise with Gold Coast-based ABI medical teams — including rehabilitation specialists, neurologists, and hospital discharge planners — to ensure that NDIS supports align with clinical treatment goals. Many ABI participants transition from hospital to community-based NDIS support, and this transition requires careful coordination to avoid dangerous gaps in care.
