What is NDIS support coordination?

NDIS support coordination is a funded service that helps you understand and implement your NDIS plan. A support coordinator connects you with providers, manages service agreements, helps resolve issues, and prepares you for plan reviews — acting as your guide through the NDIS system. It is funded under the Capacity Building budget, not Core Supports.

What's the difference between support coordination and specialist support coordination?

Standard support coordination helps most participants implement their plans and connect with providers. Specialist Support Coordination (SSC) is a higher-intensity service for participants with complex needs — for example, those with mental health conditions, involvement with the justice system, or homelessness. SSC must be delivered by a registered provider with specialist qualifications. ConnectCare Gold Coast provides standard support coordination.

Can I use ConnectCare with any NDIS plan management type?

We work with self-managed and plan-managed NDIS participants, and we can invoice your plan manager directly so there's no paperwork at your end. Agency-managed plans have different rules about which providers can be paid from your funding, so give us a quick call and we'll look at your plan with you and explain your options clearly, with no pressure.

How do I know if I have support coordination funding in my plan?

Check the 'Capacity Building' section of your NDIS plan document. Support coordination will appear as a line item under that budget category. If you're not sure, log into your myplace portal and review your plan details, or contact your Local Area Coordinator (LAC) who can explain what's included.

Do I need to be an NDIS participant to use ConnectCare?

No. Most of our clients are older people who pay privately — you choose the hours and we send the same support worker each visit, with no assessment and no waiting list. We also work with self-managed and plan-managed NDIS participants. If you're not sure which applies to you, please call us and we'll walk through it.

How is ConnectCare structured as an NDIS service?

ConnectCare Gold Coast is a direct provider. We engage our own support workers, hold the service agreement with you, and invoice you or your plan manager directly. We work with self-managed and plan-managed participants across the Gold Coast and Northern NSW. Our team is happy to talk you through how billing would work in your situation.

We service the entire Gold Coast region and Northern NSW — from Coomera and Helensvale in the north, through Southport, Surfers Paradise, Broadbeach, Robina, Burleigh Heads, Palm Beach, and Coolangatta, and across the border into Tweed Heads, Kingscliff, Pottsville, Ocean Shores, Ballina, and Byron Bay.

How quickly can I get started?

What does it cost to use ConnectCare?

Pricing for any supports arranged through ConnectCare aligns with the current NDIS Price Guide. There are no hidden fees and no lock-in contracts. The way invoices are paid will depend on your NDIS plan management type — for example, plan-managed participants typically have invoices paid by their plan manager, while self-managed participants pay directly and claim reimbursement. We'll explain exactly how billing will work in your situation before any service starts.

Can I keep my existing support workers and add ConnectCare as my coordinator?

Yes — absolutely. If you already have support workers you're happy with, ConnectCare can step in purely as your support coordinator, managing your plan, service agreements, and provider relationships. You don't need to change any of your existing workers to use us as your coordinator.

How do I pay for services arranged through ConnectCare?

Payment depends on how your NDIS plan is managed. Plan-managed participants typically have invoices paid by their plan manager from their NDIS funds. Self-managed participants generally pay directly and claim reimbursement from the NDIS. All pricing aligns with the current NDIS Price Guide. We'll confirm the exact arrangement with you before services begin.

Is there a contract or minimum commitment?

No lock-in contracts. You can adjust or stop services at any time with reasonable notice.

Do you service outside the Gold Coast?

Yes — we service Northern NSW including Byron Bay, Ballina, Ocean Shores, Pottsville, Tweed Heads, Kingscliff, Murwillumbah, Banora Point and surrounding areas — all the way from Byron Bay south to Coomera north.

What happens at a plan reassessment?

A plan reassessment (or plan review) is a meeting with the NDIA to review your current plan and set your funding for the next period. You'll discuss your goals, what supports have worked, what hasn't, and what you need going forward. ConnectCare can help you prepare — gathering evidence from your providers and building a clear case for the funding you need.

How do I change my support coordinator?

Check your current service agreement for any notice period (usually 2–4 weeks), then give written notice to end the arrangement. If you're plan-managed, notify your plan manager. Update your service bookings in myplace, and brief your new coordinator on your plan and existing providers. ConnectCare makes the transition easy — we handle most of the onboarding process for you.

What is a service agreement?

A service agreement is a written agreement between you and a provider (like ConnectCare) that outlines what services will be provided, at what frequency, at what rate, and under what conditions. It protects both you and the provider. Always read your service agreement carefully before signing — particularly the notice period and cancellation terms.

Can I use multiple NDIS providers at once?

Yes — you can have as many providers as your plan supports. Most participants use several: a support coordinator, one or more support workers, allied health providers (OT, physio, speech therapy), and perhaps a plan manager. ConnectCare can coordinate and manage your relationships with all of your providers.

How are support workers on ConnectCare verified?

Every support worker in the ConnectCare network must provide: a valid Australian Business Number (ABN), a current NDIS Worker Screening clearance, a National Police Check, and current Public Liability Insurance. Personal care workers must also hold a Certificate III in Individual Support or equivalent. We verify all documents before any worker is placed with a participant.

What qualifications do your support workers have?

All workers hold the mandatory compliance documents (NDIS screening, police check, ABN, insurance). Personal care workers additionally hold a Certificate III in Individual Support (Disability) or equivalent. Many workers also hold First Aid certificates, and some have specialised qualifications in mental health, aged care, or allied health assistance. Specific qualifications are available on request for any worker.

Can I choose my own support worker?

Yes — ConnectCare matches you based on your needs, preferences, location, and personality. If a match isn't right, we'll find you someone else. There are no lock-in arrangements and your comfort and satisfaction with your worker is always the priority. Some participants also request workers with specific gender, language, or cultural background preferences, which we do our best to accommodate.

Are your support workers qualified?

Every worker we coordinate holds a valid ABN, current NDIS Worker Screening clearance, National Police Check, and Public Liability Insurance. Personal care workers also hold a Certificate III in Individual Support or equivalent.

What if I am not happy with my support worker?

We will find you a replacement. There are no lock-in contracts and your satisfaction is always our priority.
