Talk to a Local CoordinatorPhysical Disability Coordination
Physical disability encompasses a wide range of conditions affecting mobility, strength, and bodily function — including spinal cord injury, cerebral palsy, multiple sclerosis, muscular dystrophy, limb differences, and chronic pain conditions. NDIS plans for participants with physical disability often include a mix of Core Supports, Capacity Building, and Capital Supports.
ConnectCare helps participants coordinate across all of these categories — ensuring support workers, therapists, and equipment providers are aligned and that funding is being used effectively.
Assistive Technology Navigation
Assistive technology (AT) is one of the most complex and underused areas of NDIS plans for participants with physical disability. Wheelchairs, pressure care mattresses, shower chairs, vehicle modifications, and communication devices can all be funded — but accessing them requires assessments, quotes, and plan approval.
- Connecting you with AT-qualified occupational therapists on the Gold Coast
- Understanding AT funding tiers (low, mid, high cost) and approval requirements
- Managing AT quotes and supplier coordination
- Advocating at plan review for AT funding that reflects your genuine needs
Home Modifications
Home modifications — such as grab rails, ramp access, accessible bathrooms, or stairlifts — are funded under the Capital Supports budget in NDIS plans. ConnectCare can refer you to local OTs who conduct home modification assessments and help you navigate the approval and builder engagement process on the Gold Coast and Northern NSW.
Personal Care Connections
Personal care is often the highest-use support for participants with physical disability. ConnectCare matches participants with qualified personal care workers who hold a Certificate III in Individual Support and have experience with the specific physical support requirements of your condition. We also prioritise consistency — matching you with the same worker for regular sessions. Learn more about our personal care support services.
