Book a Free ConsultationWhat Is NDIS Support Coordination?
Support coordination is a funded NDIS service that gives you a dedicated person in your corner — someone who understands the National Disability Insurance Scheme inside out and can translate your plan into real, working support. A support coordinator helps you understand the funding you've been allocated, connects you with the right local providers, and makes sure everything runs smoothly from day one. It is the single most useful service for participants who feel overwhelmed by the system.
Without support coordination, many participants receive their NDIS plan and simply don't know where to start. Funding categories are confusing, the provider landscape is fragmented, and the pressure of getting it right — within a 12-month plan period — is significant. A coordinator removes that burden. We are the bridge between your plan on paper and the real-world supports that improve your life.
ConnectCare provides NDIS support coordination across the Gold Coast and Northern NSW — from Coomera and Helensvale in the north to Coolangatta and Tweed Heads in the south. As a support work connection platform, we move quickly and work personally with each participant and family. Eligibility and funding options depend on your individual NDIS plan management type — we'll explain clearly what applies to your situation. No bureaucratic delays. No long intake queues. Just an experienced local coordinator working alongside you on your goals.
NDIS Terms Explained Simply
- Support Coordination (CB Supports)
- A line item in your NDIS plan that funds a coordinator to help you understand and use your plan. Categorised under Capacity Building.
- Self-Managed
- You receive your NDIS funding directly and pay providers yourself. Maximum flexibility — you can use any provider, registered or not.
- Plan-Managed
- A plan manager pays your providers on your behalf using your NDIS funding. You can still use any provider, registered or not.
- NDIA-Managed (Agency-Managed)
- The NDIA pays providers directly. You can only use NDIS-registered providers — the most restrictive option.
- Reasonable & Necessary
- The legal test the NDIA uses to decide if a support is funded. It must relate to your disability, represent value for money, and help you reach your goals.
- Plan Reassessment / Review
- The scheduled meeting where your NDIS plan is renewed, usually annually. Your coordinator helps prepare evidence of progress and updated goals.
Who Needs NDIS Support Coordination on the Gold Coast?
Support coordination is especially valuable if you fall into one of these groups:
- Participants new to the NDIS who aren't sure how to activate their plan
- People with complex support needs across multiple providers and funding categories
- Self-managed and plan-managed participants who want expert local guidance
- Families and carers managing the coordination burden on top of everything else
- Participants who've struggled to find reliable support workers in the Gold Coast area
- Anyone approaching a plan review who wants their funding to reflect their real goals
- Participants transitioning from hospital, school, or another provider
- People who've had a poor experience with their previous support coordinator
Support coordination is listed under Capacity Building in your NDIS plan. If it's already in your plan, the NDIA has formally recognised that you would benefit from having a coordinator — and you should use the funding. Leaving it unspent doesn't reduce your future plan; what it does is leave you trying to do a complex job alone.
How ConnectCare Provides NDIS Support Coordination — Step by Step
Our coordination process is structured, transparent, and built around your goals — not ours:
- Free consultation — we review your current NDIS plan, goals, and the supports you're entitled to access
- We map out the right service mix to meet your goals within your funded categories
- We identify and contact suitable local Gold Coast providers — support workers, therapists, programs
- We coordinate service agreements, bookings, and introductions so you don't have to chase paperwork
- We monitor service delivery, troubleshoot issues, and adjust your supports as your needs change
- We prepare you for your next plan reassessment with clear evidence of progress and emerging needs
We work with participants across Southport, Robina, Burleigh Heads, Tweed Heads, and everywhere in between. Local knowledge matters when you're coordinating in-home care, community access, daily living support, transport assistance, or SIL arrangements — because the right provider in Coolangatta is not the right provider in Helensvale.

What a ConnectCare Coordinator Does Day-to-Day
Beyond plan setup, a coordinator's day-to-day work includes liaising with providers when issues arise, problem-solving service gaps, attending meetings with allied health professionals, drafting reports for plan reassessments, and keeping a close eye on your funding utilisation so nothing runs out unexpectedly. We also coordinate community participation programs, in-home care rosters, transport assistance bookings, and respite arrangements.
For participants with autism, psychosocial disability, mental health support needs, physical disability, intellectual disability, or acquired brain injury, the coordinator role is particularly important. Each disability type comes with specific provider requirements and service patterns — and we match you to coordinators who understand your situation. Inclusive of Different NDIS Plan Arrangements
ConnectCare helps connect participants, families, and support workers across different NDIS plan arrangements. Eligibility and funding options may depend on your individual NDIS plan management type and support requirements — and we'll always be upfront about what's possible in your specific situation before any service starts.
If your plan is plan-managed, we typically liaise directly with your plan manager and handle invoicing — you don't need to do anything extra. If you're self-managed, we work with you directly and keep administration as light as possible. Whatever your arrangement, our priority is giving you choice and control over who supports you and when.
For participants in Tweed Heads and across Kingscliff and Northern NSW, working with a local platform that understands the cross-border nature of the region is a significant advantage. We also coordinate the full range of NDIS provider connections, plan navigation, and capacity building.

Frequently Asked Questions
Can ConnectCare help me with my NDIS plan?
ConnectCare is a support work connection platform helping NDIS participants, families and support workers across the Gold Coast and Northern NSW. Eligibility and funding options may depend on your individual NDIS plan management type and support requirements — different plan types have different rules about which providers can be paid from your funding. Give us a quick call and we'll explain exactly what's possible for your situation.
How much does NDIS support coordination cost on the Gold Coast?
Support coordination is funded through your NDIS plan under Capacity Building — Support Coordination. If this line item is in your plan, you pay nothing out of pocket. ConnectCare invoices your plan manager (or you directly if self-managed) at the NDIS price-guide rate.
How quickly can ConnectCare start providing support coordination?
In most cases we can begin within a few business days of your initial consultation. We don't have a waitlist for support coordination on the Gold Coast or Northern NSW. Once we've reviewed your plan and signed a simple service agreement, we get to work immediately.
What's the difference between Support Coordination and Specialist Support Coordination?
Standard Support Coordination (Level 2) suits most participants. Specialist Support Coordination (Level 3) is for participants with high or complex needs that require a coordinator with specific qualifications — typically allied health backgrounds. ConnectCare provides Level 2 coordination across the Gold Coast.
Can I change my current NDIS support coordinator to ConnectCare?
Yes — and it's straightforward. You're entitled to choose your coordinator and you can change at any time. We'll guide you through ending your current service agreement and starting a new one with us. No NDIA approval required.
Do you support participants in Tweed Heads and Northern NSW?
Yes. ConnectCare provides NDIS support coordination across the Gold Coast and into Northern NSW — including Tweed Heads, Kingscliff, Banora Point, Murwillumbah and Ocean Shores. Our coordinators know both the QLD and NSW provider landscapes.
